Virtually Better, Inc. (VBI), utilizes the services and technology from www.heroku.com (PaaS) to

provide the DentalFearless mobile app. DentalFearless is developed for research use by New York

University and is collectively managed between Virtually Better, Inc., and researchers at New York

University (NYU). NYU and VBI ("us", "we", or "our") operate, manage the content, and support the

mobile app and related websites.

DentalFearless does not require access to camera, microphone, speech recognition, face

scanner, location, photos, calendar, contacts, Bluetooth, or other functions of your phone.

Personal Information Collect, Use, Disclosure and Consent

The only Personal Information required for use of DentalFearless is for purposes of password

protection. This occurs in the form of you providing an email address to use the dashboard or

mobile app platform. Personal information in this document includes: EMAIL ADDRESS.

As DentalFearless is being developed for use in a research protocol and access to its use is subject to

participation within the larger research protocol.

This page informs you of our policies regarding the collection, use and disclosure of the EMAIL

ADDRESS you provide in order to use DentalFearless.

We will not use or share your EMAIL ADDRESS with anyone except as described in this Privacy Policy.

We will only collect an EMAIL ADDRESS such that is necessary to fulfill the following purpose:

We will not sell your EMAIL ADDRESS to other parties. We retain your EMAIL ADDRESS for the time

necessary to fulfill the identified purposes.

We are committed to ensuring the security of your EMAIL ADDRESS and may use passwords,

encryption, firewalls, restricted employee access or other methods, in our discretion. We will use

appropriate security measures when destroying your email information such as shredding

documents or deleting electronically stored information, at our discretion.

We may collect information that DentalFearless sends the third party data server whenever you use

DentalFearless ("Log Data"). This Log Data may include information such as your use of the

DentalFearless mobile app, the time and date of your use, the time spent in the app and other

statistics.

We may employ third party companies and individuals to perform DentalFearless-related services or

to assist us in analyzing how DentalFearless is used.

These third parties do not have access to your EMAIL ADDRESS.

We may use your EMAIL ADDRESS for you to sign up to use DentalFearless and to contact you only in

the event that you initiate a reset of your password.
